Delirium is the number one complication of older hospitalized patients yet often is unrecognized by providers. Clinical trial evidence is compelling that 30-40% of delirium cases are preventable. The purpose of this webinar is to review a hospital system’s interdisciplinary structured model of care for identifying patients at risk for delirium and implementation of interventions for prevention and management.
